I authorize the VES Organizing Committee to represent me as a Quality Control Analyst employed by MAXIMUS Federal Services, Inc. for the express purpose of collective bargaining, including petitioning the National Labor Relations Board for a secret-ballot election. This authorization is valid for twelve months from signature.

This is an electronic authorization and showing-of-interest submission supporting an NLRB election petition. It is not the election itself. It does not authorize dues and does not make you a union member.

Eligible employees: Current full-time or part-time Quality Control Analysts employed by MAXIMUS Federal Services, Inc., Employees assigned to Training or Special Teams remain eligible if their official job classification is Quality Control Analyst.

Excluded: Supervisors, Managers, Directors, and anyone employed by Blue Lake, LLC.

The unit is nationwide and remote. Use the state where you live and perform your remote work. Do not enter an employee ID, date of birth, Social Security number, or other sensitive identifier.

Electronic record: The form will record the submission date and time with the entry. After submission, a confirmation containing your name, personal email, telephone number, employer, exact authorization language, and submission date will be sent promptly to your personal email address.
